COVERAGE A
Benefits are available after 90 days of coverage.

$15 copayment per examination - 100% of the maximum allowable charge

• Two routine periodic examinations in any 12-month period.
• Bitewing X-rays once in every 12-month period.
• Full mouth X-rays once in every 36-month period.
• Topical fluoride application for dependent children under age 19, once in any 12-month period.
• Prophylaxis and periodontal maintenance, not to exceed two treatments per year.

COVERAGE B
Benefits are available after 12 months of coverage.

$15 copayment for each of the following services - 100% of the maximum allowable charge

• Sealants on the occlusal biting surface of first and second permanent molar teeth on dependent children under 16 years of age. Only one sealant benefit allowed on each tooth per lifetime of coverage. (copay per tooth)

$25 copayment for each of the following services - 100% of the maximum allowable charge

• Emergency treatment for relief of pain (copay per procedure)
• Restorative services; filling material such as amalgam, synthetic porcelain and composite restorations limited to one restoration per surface per tooth per year (copay per surface per tooth)
• Oral surgery: routine extractions (non-impacted), including pre- and post-operative care (copay per tooth)
• Repair of full and partial dentures (copay per procedure - top and bottom dentures are considered separate procedures)
• Stainless steel crowns (copay per tooth)
• Bridge repair (copay per procedure)

$75 copayment for each of the following services - 100% of the maximum allowable charge

• Endodontics - root canal treatment (copay per tooth)
• Periodontics - treatment for diseases of the gums and bones supporting teeth (copay per procedure)
• Surgical extractions (impactions) (copay per tooth)
• Space maintainers for eligible dependents up to age 14 (copay per procedure)
• Relining and rebasing of full and partial dentures limited to one upper and one lower every three years (copay per upper and copay per lower)

COVERAGE C
Benefits are available after 24 months of coverage.

$75 copayment for each of the following services - 50% of the maximum allowable charge

• Full and partial upper and lower dentures. Payment includes any necessary adjustments for a six-month period. (copay per upper and copay per lower)
• Initial placement of fixed and removable bridges by standard procedures (copay per tooth)
• Cast crowns for the treatment of cavities or fractures when the teeth cannot be restored with amalgam, synthetic porcelain or composite restorations (copay per tooth)
• Cast onlays (copay per tooth)
• Laminate veneers (copay per tooth)

After you pay the applicable copayment amount, TRH Dental Care pays 100 percent of the maximum allowable charge for Coverage A and Coverage B services. TRH Dental Care pays 50 percent of the maximum allowable charge for Coverage C services after your $75 copayment. If you use an out-of-network provider, you could incur charges that are over the maximum allowable charge.

Orthodontic services and supplies are excluded from this coverage. Annual maximum for all coverages is $1,500 per person. This information is an overview of the TRH Dental Care coverage.
